HFD Practice Referral Program 

Program Effective Date: 11/30/2025

Program Summary: Earn up to $2,000 when you refer a new practice to HFD. Rewards are based on the referred practice’s total patient financed volume during their first 60 days after enrollment.

By participating in the HFD Practice Referral Program (“Program”), you agree to these Terms and Conditions. Healthcare Finance Direct, LLC (“HFD”) reserves the right to modify, suspend, or terminate the Program at any time.

Program Overview:  The HFD Practice Referral Program (the “Program”) is offered by Healthcare Finance Direct, LLC (“HFD”) and provides eligible participants (“Eligible Referrers”) the opportunity to refer eligible dental practices (“Referred Practices”) to HFD’s financing platform.

Program Details: HFD will provide the following rewards (“Rewards” or “Your Payout”) in the form of digital gift cards to an Eligible Referrer who refers a Referred Practice, in full compliance and in accordance with these Terms and Conditions set forth herein (such qualified referral, an “Eligible Referral”):
 

Rewards will be issued to the Eligible Referrer within one hundred twenty (120) days after the Referred Practice’s first 60 days of financed volume are completed, provided all Program conditions are met.

Reward Eligibility Requirements: Rewards are issued only if all the following conditions are satisfied:
  • The total financed volume used to determine the Reward amount is based on the first sixty (60) days following the Referred Practice enrolling with HFD.
  • For multi-location practices, only the single location with the highest total financed volume in the first 60 days will be used to determine the payout. Rewards are not issued per location under the same ownership.
  • The Referred Practice must complete enrollment within sixty (60) days of first being referred to HFD through the Referrer’s unique referral link.
  • The Referred Practice must remain in good standing and show no signs of misuse or fraudulent activity, including patient payment issues such as past-due accounts or other irregular activity.
  • The Referrer must be in good standing with HFD at the time of payment
  • The Program is valid only within the United States.
HFD reserves the right to modify, suspend, or terminate any aspect of the Program, including the Reward structure, at any time. All HFD decisions regarding Program administration are final and binding.

Participant Eligibility: To qualify as an Eligible Referrer, you must:
  • Be actively enrolled in and in good standing (i.e., not suspended or terminated) in the Program at the time the Referred Practice is in any part of the onboarding process with HFD.
  • Participants may not refer themselves or any practice or business they own, operate, are employed by, or are affiliated with under common ownership. Self-referrals are strictly prohibited.
  • New, existing, and former HFD practices are prohibited from referring themselves (i.e. self-referrals), any businesses they own, or any affiliated location under common ownership that they themselves are a part of. Referred Practices include the owner of the practice, the practice employees, and anyone with an employment relationship with the practice.
Referred Practice Eligibility: To qualify as an Eligible Referral, the Referred Practice must:
  • Not currently, and never have been, an HFD provider.
  • Not already exist as a lead or contact within HFD’s internal systems.
  • Successfully enroll and onboard as an HFD provider within sixty (60) days of being referred.
  • Remain in good standing and compliant with HFD policies during the evaluation period.
Prohibited Conduct: Eligible Referrers agree not to engage in any conduct that may harm HFD’s brand, reputation, or interests, including (but not limited to):
  • Attempt to earn Rewards through fraudulent, misleading, or unethical means.
  • Violate the intent or spirit of these Terms and Conditions.
  • Use unauthorized marketing, spam, or other deceptive tactics.
How to Register and Earn Rewards:  
  • Complete the registration process at refer.gohfd.com.
  • Use your unique referral link to refer practices. IMPORTANT: Only Referred Practices that complete the Referral Demo form through your unique referral link will be eligible for Reward/Payout.
  • Referred Practices that do not complete the Referral Demo form will not qualify as Eligible Referrals and will not be tracked or credited.
  • Eligible Referrers will receive their payout in the form of a prepaid Visa® or Mastercard® e-gift card.
Rewards Issuance: Rewards will be distributed electronically to the Eligible Referrer’s email address on file with HFD. Rewards are considered earned, payable, and reportable for tax purposes once made available to the participant, regardless of whether they are redeemed or used. If the total value of rewards made available to a participant equals or exceeds $600 in a calendar year, HFD may be required to report such rewards to the IRS in accordance with federal tax regulations. Therefore, all participants must first complete an IRS Form W-9 during registration. Rewards may not be transferred, substituted, or exchanged except at HFD’s sole discretion.

General Conditions: By participating in the Program, participants agree to:
  • Release, discharge, and hold harmless HFD, its affiliates, officers, directors, employees, and agents (collectively, the “Released Parties”) from any and all liability or damages arising from participation in the Program or use of any Reward.
  • Not represent yourself as an agent or employee of HFD, or as having authority to bind HFD in any manner.
  • Promote HFD only in a truthful, lawful, and compliant way.
HFD is not responsible for lost, delayed, incomplete, or misdirected referrals or payments due to network, system, or technical errors.
 
HFD may modify, suspend, or terminate the Program at any time, with or without notice. Updates will be posted on refer.gohfd.com. Your continued participation constitutes acceptance of any modifications.

Disqualification and Termination: HFD reserves the right to disqualify or deregister any participant who violates these Terms and Conditions. Upon disqualification, the participant shall forfeit all Rewards, whether issued or unissued.

Use of Name and Likeness: By participating, you grant HFD permission to use your name, image, and statements about the Program for marketing or promotional purposes, in any media, worldwide, without additional compensation, unless prohibited by law.

HFD Trademarks and Brand Use: You acknowledge that all HFD names, logos, trademarks, and related branding (“HFD Marks”) are the exclusive property of Healthcare Finance Direct, LLC.

You may use HFD Marks only with prior written approval from HFD and in accordance with brand guidelines. All HFD Marks remain the sole property of HFD and may be revoked at any time.

Limitation of Liability: HFD’s total liability for direct damages under this Program shall not exceed $1,000. HFD and its affiliates shall not be liable for any indirect, incidental, special, punitive, or consequential damages, including loss of revenue or goodwill, even if foreseeable.

By participating in the HFD Practice Referral Program, you acknowledge that you have read, understand, and agree to these Terms and Conditions.
